Setting up Surveys and Lead Capture forms on my Prize Wheel

Need to capture data from your participants? Here's how!

You can collect player data in two different ways by using Surveys or Lead Capture forms.


Adding a Survey

To set up and attach a Survey to your Prize Wheel navigate to Edit > Include a Survey > Create Survey and follow these steps:

  1. Click on the blue Add button

  2. Give it a Name, Title, and Description. Remember that the information on the Name field does not show up to your participants.

  3. You can make fields required by checking the box under the Required column.

  4. Add new fields with the blue Add Survey Field button and select a field type from the drop-down menu. There's a ton of options available!

  5. Navigate to Prize Manager > Edit and turn on the Include a Survey option.

  6. Select the Survey you've just created from the menu and click on Update to save your changes.

Once you've added the Survey to your configuration, it will show up on your Prize Wheel for attendees to fill out.

To gather Survey data, simply navigate to Manage > Download to download your data.


Adding a Lead Capture form

Create a Lead Capture form and attach it to your Wheel by navigating to Edit > Include Lead Capture Form > Create Lead Capture and follow these steps:

  1. Click the blue Add button.

  2. A Lead Capture form will be created with First Name, Last Name, and Email as the default fields. Feel free to add and rename the fields to best fit your use case. Hot Tip: anything you enter into the Name field won't be visible to players!

  3. Click on Create

  4. Navigate to Prize Manager > Edit and turn on Include Lead Capture Form

  5. Select the form you've just created from the drop-down menu. Scroll down and click on Update to add the form to your Wheel.

To download your Lead Form Data, navigate to Manage > Download to download your lead data.
