If you've already created a Survey, you can add it by following these steps:
Trivia:
Trivia > Configurations > Game Name > Survey (Optional) > Dropdown
Prize Wheel:
Main Screen > Prize Manager > Configurations > Prize Wheel Name >Game Setup > Turn on the Include Survey button.
Digital Fishbowl:
Main Screen > Configurations > Fishbowl Name > Screenflow > Survey
